Procedure Request Forms

Before you submit a request form, note that you need a valid animal use protocol number. Please visit the Office of Research: Animal Care and Use Committee webpage for information about obtaining a protocol number if you don't already have one.

Click on the links below to download a copy of the appropriate request form(s). The request forms can be completed on your computer and printed, or you may print out blank copies and complete them by hand.

The forms must be submitted on paper; please do not e-mail completed forms.
