Community Physicians
Who are we?
Wake Forest University Baptist Medical Center Community Physicians was formed in 1994 as a group of neighborhood health care practices in an effort to stabilize and expand cost affordable, high quality, primary care services, into communities located throughout northwestern North Carolina.
Who will care for me?
Community Physician practices are staffed by Board Certified Family Physicians, Internists, Pediatricians, Nurse Practitioners and Physician Assistants -- allowing us to care for and meet the needs of the entire family. As an affiliate of Wake Forest University Baptist Medical Center, our physicians, mid level practitioners, and support staff must meet the high standards established by the Medical Center to become a part of our health care team – your assurance of quality health care. We also have access to the renowned specialty expertise and leading technology that the Medical Center has been providing to our communities for more than half a century.
What services are provided?
The practices provide a wide range of services from general medicine to patient education. Our providers enjoy the full range of primary care and have special interests in sports medicine, diabetes, hypertension, women’s health, disease prevention, dermatology, nutrition, wound care, geriatrics and more. Our entire health care team places strong emphasis on four key elements of a successful health care system:

Where are we located?
There are currently Community Physician practices located in five counties throughout northwestern North Carolina to include Forsyth, Davie, Stokes, Surry and Wilkes.
